Yes, you can, just bring up the screen and hide the keypad if it is on the screen, then on the same screen where you have the hold button, the speakerphone button, the button if you want to add another caller and so on find the "Facetime Video" button and tap it. There might be a very small disruption, but basically what happens is that while you are still speaking to the other person they are being notified that you are requesting a Facetime call and they can accept it at which point the regular call ends and the Facetime Video call starts; it's quite seamless and I have done it many times with family and friends.
