ON the iPhone, Settings, Accessibility, Spoken content. Speak selection will speak text you highlight.
Speak Screen will read everything when you swipe down with two fingers from the top of the screen, i.e. the status bar. This has nothing to do with VoiceOver, this is the kind of speak on demand you are looking for. You can set the voice and the speaking rate in that section as well. The one advantage this has over VoiceOver is that once you start it speaking, like a very long email, if you lock the phone, it keeps speaking; or at least used to in earlier versions. I haven’t tested it in the last couple iOS versions since I’m retired and not needing to teach it to low-vision users. Her vision is not affected per sea. But her cognitive processing of written language is very limited. Anything longer than a short text message “overwhelms” her brain. She uses an iPhone. She is asking for an iOS feature or add-on apps that use text-to-speech to read out on demand more sizeable amounts of text such as an email, longer texts, text on a webpage, etc?
