Screen recognition does exactly what it is supposed to do when it is needed. 
When it is not needed which means in apps which are accessible, it can make 
things look weird which is why one should make sure that it is turned off for 
all apps unless you know for sure it is required.
In Settings > Accessibility > Voiceover > Voiceover Recognition you should turn 
it on, but then go to the app specific listing and make sure it is not selected 
for any apps.
Now add it either to the rotor or to Voiceover Quick Settings which is done in 
exactly the same way, either go to Settings > Accessibility > Voiceover > Rotor 
or Settings > Accessibility > Voiceover > Quick Settings, make sure Screen 
Recognition is selected and you can also then drag it into a position of your 
choosing.
Now if you are in an app where things aren’t as they should be and you want to 
try to see if it helps to use screen recognition you can either turn the rotor 
to Screen Recognition and turn it on or bring up Quick Settings and turn it on.
If you find a particular app definitely benefits from it you can go back to 
Settings > Accessibility > Voiceover > Voiceover Recognition and select that 
particular app in the app listing and whenever you open this app screen 
recognition will be active, but it will not be so once you close the app or go 
to a different app.
