I just use Reminders, and I usually tell Siri, set reminder for 9 a.m., specify date and say what I need at what time. It works pretty well.

Or, what I usually do, is Use Siri to set the appointment, then go into the appointment details and double tap on Edit, then on Alert then flick through the choices and remember to hit done when finished. Or, you could go into settings, calendar, and set a default alert time for all new appointments.
