Do a triple tap on the App store icon. From the menu that comes up, double tap on purchased. Now, select my purchases assuming the card is in your name. IN the search field enter the name of the app. Flick to the right to find it and it will give you at least the date you got it, and I think it may show the price, but having the date, you could call your credit card people and have them look it up if needed.
And, if you can show it is for blind, in some States and I'm pretty sure at the federal level it may be deductible.
