I think you have to go in to the health app on your phone to set it up. I
did this such a long time ago, I barely remember.

I think it is more a feature of the OS, not the version of watch. I have
a series 6 running OS 9.2 and it works for me.
After starting the ECG app, touch near the bottom of the screen.
It will first say, hold your finger on the crown, pause for a second, then
say, magic tap for live audio.
On that screen, if you do the magic tap, it will either say live audio on,
or live audio off. The magic tap is a toggle.
