Actually I just had the same issue yesterday. I was trying to take an Uber to
church, and definitely had the church as a saved place, but I had to reenter
the address. And it didn’t have my home address anymore either. Glad I’m not
the only one. I don’t use Uber often so I thought maybe I was doing something
wrong.

somehow after a week my Uber has changed. when I open "where to" It no
longer shows my home address in the pickup field. I have more than twelve saved
places but it only shows five and they are ones I haven't gone to in months.
yesterday I was going to my daughter's house and it would not accept her
address. When I entered the same information on my wife's iPhone it took it
with no problem.
Note: I did discover that in the Activities tab it showed previous trips I
could rebook. When I double tapped the bowling alley that I go to weekly
everything was correct but my home address.
