One good option is mentioned above. It has been a gold standard for years. One thing that hasn't been mentioned is that it now costs 80$/year (comparing to 30$/lifetime that it has cost last year or 15$/lifetime that it has cost for the largest part of its history). My app Speech Central is completely free when used with VoiceOver.
