Also, do you have any Bluetooth speakers or headsets you use? Make sure one of them hasn't gotten turned on so Voiceover might be coming through it and you don't notice it. It has happened to me with my Sleep Phones where I had them in my backpack and they turned themselves on because the button got pushed inside the pack and I was wondering why I didn't get anything out of my iPhone. If that is a possibility pressing and holding the side button and asking Siri to turn off Bluetooth would be a way to make sure this is not the problem.
