Or you could just set your reminder for the time you want to first be alerted
and include the actual appointment time within the text of the reminder.

Custom – this allows you to select, for example, 3 hours or 5 hours
before, but you can only do full hours, something like 4 hours and 30 minutes
before is not supported.
I have so far not found a way to set a reminder with Siri for a
particular time and include the information about a 2 hour early reminder.
Maybe once we get further with Apple Intelligence Siri will be able to do this.

Once I asked Siri to remind me and the reminder showed up in my windows
pc instead of the phone.
I would like the phone ring or make any sound to remind me
