I do not know of another way to change voices while in a document. But as for accessing your downloaded voices from the Audio Settings in a book; double tap on audio settings, then on voices, then touch at the bottom of the screen and double tap on Downloaded. Then your downloaded voices will be after the annoying push to pay for the extra voices...
