Rina, In Settings > General > Accessibility > Voiceover is an option for a Voiceover practice area. This allows you to practice gestures and anything you do will give you feedback, e.g. if you want to do a 2-finger double tap and it says 2-finger triple tap it means you tapped 3 times instead of twice. You can also practice the rotor gesture here and I would encourage you to do so because using the rotor is essential and it is just as important to know how to use the rotor as it is how to do the various 1-finger, 2-finger and 3-finger taps.

You should be able to turn the rotor to links, flick down and double tap on the link. Depending on what program the doctor's office uses, you may have to give permission for it to access the camera and the microphone. If it gets too coplicated, call the doctor's office and request a regular phone call. I had a problem connecting to my doctor once and he just called me on the phone.
