Rob, Use your earpiece, then make Voiceover talk continuously, for example, put focus on the top left app on your home screen and do a 2-finger swipe down to get Voiceover to start to read all the apps. Once it is reading, raise the volume on your iPhone's volume button and see if that will do the trick. The fact that the iPhone volume is at 100% does not mean Voiceover volume is as Voiceover is treated separately. Alternatively, you can also make sure that you add volume to the rotor, then flick the rotor to volume and do a single finger swipe up and see where the volume is and if you can raise it.
If you ever want to make SIRI lowder or quieter you have to do it the same way, make it talk by asking it a question which involves a longer answer, like a math question or something, then when SIRI is talking you can adjust SIRI volume with the volume buttons.
