Hi, I use HippoRemote to control my media Centre computer. It may not be exactly what you are looking for, as I do need to be close enough to the media Centre to be able to hear the screen readers speech through the computer itself. HippoRemote does not bring speech to the device but does allow me to totally control the computer quite well from a distance.
