If you're familiar with the mobile web interface to Gmail then I think this app will look familiar to you.
It does seem to work well with VoiceOver. I haven't used it much, but you can check each message and then hit a delete button. I haven't come across a delete all button, but I haven’t used it much, and I haven't looked for such a button in Gmail or the built in mail app either for that matter.
