I know this does not help the immediate concern, but if you are jailbroken there is a tweet that you can install so you can enable or disable accessibility features through other means. For example I have mine set so pressing both volume up and down will turn voiceover on or off. This is my backup in case triple press of home doesn't work for some reason.
