It is possible, but the usual causes are: the iDevice is somewhat tilted from the plain of the item (the iOS device must be level to the item being photographed / scanned), the iDevice is too close or too far from the item (most people err on the side of being too close), the camera is being physically blocked (the camera and flash are in the upper right corner of the back of the iPhone when the iPhone is facing you with the home button on the bottom), and/or the iDevice is moving whilst the picture is being taken (any movement, even the smallest twitch, will blur the image, making it difficult or impossible to be recognised).
