Ok, this is such an obscure situation that nobody has reported it on any list 
which I am on until now. Therefore, it took me a while to figure out what is 
going on. Basically, even though you have your vibrate set to on for both ring 
and no sound, your vibrate for phone ringer is turned off. You can turn the 
vibrate for any specific notification on, set to any pattern, or off.

Go to settings, sounds. Go down to sound and vibration. The first option is 
ring tone. Activate this option. Your first or second option should say 
Vibration None. Activate this option. At the top of the list is the option 
Alert Default. Activate this option. What default means is, when the iPhone is 
set to factory defaults, vibration Alert is the automatically selected choice. 
Therefore, it was changed to none by someone, either accidentally or on 
purpose, after the iPhone was set up. The cool thing here is, you have a choice 
of different vibration patterns to choose from, or you can create your own 
pattern. And, this can be done with any of the vibration alerts. So, if someone 
is hard-of-hearing, or in a very loud environment, the vibration pattern can 
signal the person as to which alert is being activated. Vibration patterns can 
also be set for specific contacts in the contact list.
