What model of iPhone are you using and which version of iOS do you have? In my experience, VoiceOver can cause a lot of peculiar behaviour with respect to the volume, but you are less likely to have problems with later versions of the software. I can't remember ever having this problem yet on my iPhone 5, but when I had an iPhone 4 it happened very frequently.
