That language setting is to do with the language that the operating system will 
be in (menus etc) and isn’t to do with VoiceOver. Because there are only two 
variations in written English (UK and US) hence only two versions offered in 
that menu. Australia and other English speaking countries adopt one or the 
other. I could be wrong but I believe that all countries that speak English 
other than the US write in British English with all it’s weird spelling foibles.

The region setting is the one that impacts the choice of VoiceOver synthesizer.
