You can try to add Volume to the rotor, then when you are on a call, turn the rotor to Volume and flick up until it is at 100%, then without Voiceover speaking, try to turn the speaker volume down using the volume down button. This worked well for me on iOS 5 and I also have a 4S so I think it's more a combination of software and hardware because ever since iOS 6 came out I can't properly balance Voiceover volume and speakerphone volume. Apple told me this does not effect all devices which of course doesn't help me much. In fact, I recently received a replacement iPhone 4S since on my original one the camera was not taking clear pictures any more and I have the same problem on the replacement 4S as I had before. Lastly, the best fix for this is simply to use headphones if you have to make a call where you have to dial a lot of numbers.
