This is the proximity sensor kicking in and out. It is default behavior when Voiceover is running that if you take your phone away from your ear that the call switches automatically to speakerphone. As it does this there is a second or maybe only half a second when it sounds to the caller that the call is interrupted. I assume you either don't hold your phone against your ear consistently, i.e. you might move it away a few inches , just far enough for the proximity sensor to think you took it away and want it to switch to speakerphone. You may also have it in a case which interferes with the proximity sensor, maybe the case covers it or is tooc lose. The proximity sensor cannot be felt, but it is right up in between the top edge of the phone and where the cut is in the lglass for the earpiece. Lastly, your proximity sensor might be malfunctioning. Apple used to have more of a problem with this a couple of years ago, but on some phones the sensor might be more sensitive than on others. If you are using a case I suggest to try it without the case, make sure you really hold your phone to your ear as in so it's touching your ear or at least extremely close, like no more than half an inch away. You also want to make sure you don't hold it too high, the part where the cut is in the glass near the top of the phone should be against your ear, not above your ear lobe. If you do all this and it still switches constantly you might have to get it checked/replaced at an Apple Store or your carrier. OK, one last comment: really? An iPhone 4S this late in the game? I would have waited just over 2 months and at least have bought the iPhone 5 after the new phone comes out in September. Of course if you needed a phone now because your old one wasn't working then that wouldn't be an option.
