Hi John, You are lucky then because it definitely can't be a non-accessibility issue. The automatic switching to speakerphone only works if Voiceover is on, if a sighted person takes his or her phone away from the ear it will not automatically switch to speakerphone, they have to tap the Speakerphone button.
When I first came on the list I think some time in 2010 and I think maybe it was after the iPhone 4 release and the iOS 4 update there were a lot of messages at one point where people had this problem.
