In IOS 6, if you muted your phone, you'd still hear VoiceOver but you wouldn't get the little sound effects. In IOS 7, if you mute your phone, you still hear voiceOver and the sound effects. I guess you have to go into Settings -> General -> Accessibility -> VoiceOver On and use the sound effects toggle.
