What happens if you do a 2-finger swipe up on the home screen to make Voiceover start reading all the apps and while it is talking repeatedly press the volume up button. iOS always has remembered different volume settings for headphone, speaker, SIRI, Voiceover etc. and to change
Any of them you have to do so while what you want to change is active. As far as I know the volume in Settings, Sounds only applies to ringer and alerts and has nothing to do with Voiceover.
