When you want to turn off your display, be sure to lock your iPhone first, then turn off the display. Likewise, you need to press the sleep / wake button before turning the display back on.
It's weird, but doing it this way all the time should prevent the necessity of having to constantly repair the phone and display. If you do things in another order, the pairing is lost.
