Hi Kathy. When you add an appointment to the calendar on your iPhone under ios 7 the alert buttons for alert 1 and alert two are toward the bottom of the screen.
You should be able to swipe right to get to each one and double tap each one to setup when you want your alerts to appear in advance of your appointments.
