You can use the custom label feature of Voiceover. Find the picture you want to label, do a 2-finger double tap and hold to bring up the custom label dialogue. Now type the name you want to give the picture in the text box and then tap Done at the bottom right of the keyboard or Save which is 2 flicks to the right of the text box.
