Many bluetooth earpieces work just fine. Voice controls are the responsibility of the earpiece, not of the iPhone or any other phone, and very few have this capability. Many people like the Aftershokz Bluez, but that is just one of many styles.
As to Voiceover saying the call's elapsed time, this is a Voiceover thing and happens with anything - bluetooth headset, wired headset, even speakerphone. The only way to fix it is to move Voiceover's focus off the time counter by touching the screen or flicking to the right, or muting speech entirely. If possible you could also lock the screen - doing so will not end your call.
