The two-finger double-tap and hold sets up to have VO state something different than what is labelled. In other words, it is the graphics labeller. So, you changed what VO says when focus is on a certain app or graphic.
The solution is to go to the now mislabelled item and do another double-tap and hold to change the babel back to what it was originally.
