Please do not take this the wrong way.

Double-tap the display with four fingers, or go to settings, general, 
accessibility, VoiceOver, practice area, and perform much more practice with 
your gestures. You really need to get to the point where you are performing the 
gestures you believe you are performing 100% of the time. By your message, it 
is clear that this is not currently the case. For instance, when you perform 
the single-finger vertical flick to change the volume, or any other flicker, 
the choice does not go up and down and up again unless your gestures are not 
precise.

The same is true with the two-finger double tap. You state you can get music to 
start and stop playing with just a few tries. I get music to start with one try 
and to stop with one try, always and every time I perform the command. This is 
because I practiced the gestures extensively until I started performing them 
100% of the time. Absolutely nothing but lots of practice will correct your 
difficulties with the gestures.

When answering and hanging up phone calls, you are most likely holding your two 
fingers too close together, so the touchscreen is only able to detect one 
finger. Because we become anxious when the gesture does not perform correctly 
the first time, the natural reaction is to tense up and cause our fingers to 
come even closer together before we try again. When the gesture continues not 
working, we do more of the same and get even more anxious.

Remember, the gesture commands do work properly, all of the time, when we 
perform the commands properly. Practice, practice, practice, and when you think 
you finally have the commands down, practice some more because the first few 
times this happens, the chances are you are bored and flustered with the 
practicing. Remember, it initially took you a long time with lots of 
frustration and many mistakes when you learned to type, and when you learned to 
use the computer with the screen reader. This is no different.
