First be sure you are in good light. Then, place the camera lens right on the rectangle holding the code, so the code is oriented such that iss left edge is toward the volume buttons. (in other words, the camera will see it the same way you would see it to read it). Then, slowly lift the phone straight up, until the code is captured. I can do this reliably on a Mac or iOS, it just takes a bit of practice. I also cannot overstate the importance of a good, strong light source, preferably not straight overhead so you don't cast as much of a shadow on the code with the phone.
