This app is awesome! I use it all the time! If you're in a room with the lights off, you'll want to turn on the flashlight feature if you have iOS 7. If you're still using iOS 6, your best bet would be to turn on a light and then start recognizing bills. If you're in a regular lighting situation, i. e. Grocery store, restaurant, outdoors, you should be absolutely fine without turning on the flash.
You don't need to do anything special to be able to recognize a bill. Just open the app, wait for VoiceOver to say "MoneyReader running," and then just place the bill on the palm of one hand, and hold the iPhone's back-facing camera over the bill with your other hand, making sure that the iPhone is at least a few inches away from the bill, and wait for VoiceOver to announce the bill's denomination. You'll want to be sure to remove the app from the App Switcher when you're through with it, as it's constantly using the video camera, which can take a large toll on your battery.
