It sounds to me like your dragging the app to quickly.  Move the app in a 
checker board pattern meaning, up or down, left or right.  Voice over will 
announce where your at as well as approaching the next page or the previous 
page.  For example, as you drag an app to the right side of the screen voice 
over will say app approaching edge of screen or words to that effect.  When you 
find where you want the app to be, just lift your finger and that's where the 
app will be placed.  Don't worry about screwing it up because you can put it 
all back the way it was in settings, general, reset, reset home screen.
