Skype is completely accessible and has been for quite some time. Yes, if you start a call it defaults to the earpiece just like the regular Phone app does. Only difference is that it behaves like the Phone app does without Voiceover which means it does not automatically switch to speakerphone when you hold it away from your ear, but the button you tap is properly labelled as "Audio, Earpiece button" and easy to find. Just touch right above the home key when the call is established and you will find the End Call button. Now swipe left about 3 times and you are on the button, double tap it and the call will be switched to speaker phone button. The button will now say "Audio, speakerphone button". If you double tap it again it goes back to earpiece. The same goes for adding video to a voice call with another Skype user. Go left again from the button I described above and you will find a button called "Camera, no video button". Double tap it and you get options to select the front or back facing camera. Skype is also the only VoIP app I know of where you can use touch dialing on the main keypad, e.g. you don't have to double tap each number to dial it. The DialPad you can bring up during a call unfortunately still requires you to double or split tap.
