This is just from memory, and I have not done it myself this way, but I thought you were able to just copy the iTunes media folder's plus the iTunes.ITL file into your new iTunes installation. I did do it by just copying the iTunes media and not that ITL file, but all that got me was having to set up all of my lists and other preferences all over again.
