Here's the list from AppleVis, which is the closest we have to an official one. 
After the link, I'll write the most severe/critical bugs.

* If you change the status cell setting in VO's braille options to anything 
other than off, you'll cause VO to completely disappear and stop working, yet 
iOS thinks it is still active. The only way to fix this is to set the status 
cell back to off.

* Scrolling is pretty bad. Flick right through an email, or have VO read it, 
and focus doesn't follow. So, if you land on an attachment, double tapping does 
nothing because focus has not followed VO. This seems to happen in webpages and 
other areas as well, but thankfully not in lists (such as mail or mailbox 
lists, Twitter apps, and so on).

* Bluetooth keyboards might duplicate or drop characters as you type. Disabling 
predictive typing could help.

* If you use the sharing options in any app to share something via 
text/iMessage, the "send" button will be present, but you can't activate it 
with VO. You have to turn VO off and hope you touch the right part of the 
screen to send the message.

* Handwriting may crash VO on some devices.

 When you dial using the keypad, whether making a call or while in a call, a 
number could get stuck. Press the number again to stop its tone from sounding, 
or (more drastically) kill the Phone app completely.
