If you are talking about 8.0.2 then officially there are no improvements. A few people have reported that Voiceover seems to crash less frequently and seems more responsive. I don't know if my full restore on Sunday helped or if it's an under-the-hood change in iOS 8.0.2, but it does appear so far that I have no more issue with DialTones sticking when I dial a phone number on the keypad. That alone is a very welcome improvement.
I think it's important we continue to send constructive emails to [email protected] asking for Voiceover fixes in iOS 8.0.3.
