What are you doing to turn it off? As Shawn pointed out, if you have SIRI you can just tell it "Turn Voiceover off" and also "Turn on Voiceover". If you want to use the triple click of the home key and it doesn't work, check in Settings, General, Accessibility. At the very bottom is a button which says "Accessibility Shortcut". Make sure it is set to Voiceover. If it is not, double tap on it and select Voiceover.
