Hi, I had the same problem.. there is a bug in visdn that needs to be fixed.. 
but in the mean time, if you change your dialplan to look like this, then it 
should work.

exten => 291,1,Wait(0.5)
exten => 291,2,Dial(SIP/291,20,rt)
exten => 291,hint,SIP/291

I got this message from Daniele:

"Can you please send a "debug visdn q931" + "debug visdn generic" log 
showing the missing alerting?"
