It's worth checking that the card is seated properly in the pci slot,
mine have popped out slightly before and I got the symptoms you
describe. I was able to push the cards down about 3mm (do this with
power off!) and then all was OK. It seems the geometry of the end plate
doesn't quite agree with the chassis they're in. It fits ok in another
pc though.
