Unfortunately, we have problems with reproducing your problem. Please
could you try to provide a reproduction script (step by step)?

What kinds of projects do you use?

Please also tell us what  "detect when file is changed outside the
environment", "auto-load changes if saved" settings do you have? To do
that select Tools->Options->Environment.documents   in Visual Studio
main menu.
