I've been re-reading the RFB 4.0 proposal that Andrew van der Stock (and others) originated, and happened to re-read section 4.2 which says:
"As of this writing, there are two conflicting versions of the 4.0 protocol, this document, and an internal-only release at AT&T Research Laboratories (ORL). Details of the ORL 4.0 protocol and the ORL 4.0 implementation have not been released."
