Assuming you can talk directly to the server's port 5900+display number,
yes.  When using a router this is normally call "Port Forwarding".
Using a proxy may not be possible, it just depends on how it is
configured.  There are some workarounds to tunnel VNC through HTTP that
should work over a proxy.

Are you having problems with your setup?  Post some details and we can
try to help you get it going.
