I think the subject summed it up, I am able to reproduce this on both Windows XP (inside VMware 4.5.2 on a Linux host) and on NT4 Server (a standalone system), with vncserver 4.0 on both, when connecting with vncviewer 4.0. If I do CTRL-ALT-DEL from the viewer through to the server and click "Cancel", it refreshes really slowly, and I lose keyboard input. Also, double-clicking on any desktop item brings up the properties for that item. This loss of keyboard exists on the affected server both through the viewer or if I disconnect and use the server keyboard itself.
The way to free the affected system of this effect is hitting CTRL-ALT-DEL on the server keyboard (not through the viewer) and click "Cancel", or hit CTRL-ALT-DEL on the server PC (through the viewer), lock it, and unlock it.
