I also using inetd method on FreeBSD and Linux.
Conmapring my '/etc/xinetd.d/vnc configuration,

':40' on server_args specifies VNC server to use specific port.
Then anothe instance cannot use such port.  Delete ':40' parameter
and try again.  Also, there should be "flags = REUSE NODELAY".
