Does your ISP assign your IP?  I would register a dynamic dns name at
www.dyndns.org and get DirectUpdate from www.directupdate.net to
ensure your dynamic dns is updated when your IP changes.  I've been
using both for quite some time now and couldn't live without them.

Also, have you forwarded port 5900 to your internal machine's IP? 
I.e.  Your router may assign an IP of 192.168.0.100 to your computer. 
You would want to forward the port for realvnc to that IP and not the
IP your ISP gave you.
