Sure the problem you are having is the following:

Traffic from "outside" hits the modem and then goes where?  The answer
is, it doesn't know where to go; when you use it for outbound stuff
(web surfing) you send info out first so it knows that when a response
comes back wich computer to tell it to go to.

Does this make sense so far...

So if you want to have outside traffic end up at a specific computer
(and program, vnc) you need to establish a port forward; i.e. you need
to tell the NAT/Router/Firewall/etc. where to send the traffic.

My guess is that the modem you have is actually a router + modem try
looking at www.portforward.com and looking for instructions for your
model.  If you cant find it let me know and we can see how to set one
up for your device.   The port forward will need the following
information (at most)

Outside Port - the number you want to type in after the IP in your
computer to connect to them e.g. IP Address:5900    (5900 is the
default for windows)

Inside IP - the inside IP to remote control (if you want both then you
need two forwards, they need to have different outside [orts)

Inside Port - the port number of the vnc application - usually this
one will be 5900.
