Don't know if this has been mentioned before or not, but the XP firewall from SP2 will block VNC unless you disable the firewall and/or allow it in the firewall. Also, if you're connecting through some sort of NAT router, you MUST port-forward port 5900 at a bare minimum. There are other ports as well, depending on if you're trying to use the Java client or do a reverse connect.
