You can use hamachi

So you install hamachi and realvnc on both machines.

This sets up a virtual network adaptor similar to the microsoft loop 
back adapter on your pc and you get a second ip address 
associated with hamachi.  You enter a network name that you 
dream up into hamachi on both machines.  It then connects those 
two machines together with an apparantly secure peer to peer 
encrypted tunnel.

VNC detects that second ip adapter and just uses it.

When I set it up about a week ago I was surprised at how easy it 
was.  The only thing is that you still have to open poerts 5800 and 
5900 on windows fire wall.  I did not have to change the adsl 
modem at either end.  One end was at my work where I would not 
have been allowed to touch the adsl modem / router.  It all ended up 
just too easy.  I installed the two programs accepting all the defaults 
and followed the set-up instructions and it work.
